Care Plans

Care plans help patients and healthcare providers coordinate care, reduce unnecessary consultations, and keep information up to date. They also help patients identify actions they can take to achieve treatment goals.

Care plans typically involves:

  •  assessment of the patient’s condition/s in relation to overall health and functionality
  • addressing the patient needs and goals
  • planning treatments and actions which will meet the goals, and planning review steps.

If your GP involves other health and care professionals, the plan will also include:

  • collaboration between providers to prepare the plan
  • regular review of the team arrangements.

Health Assements

One of the most effective ways for us to provide health promotion and prevention services in General Practice.

Our health assessments are performed by practice nurses working alongside our skilled GP’s.

A health assessment provides opportunity for your GP to examine lifestyle habits to identify possible improvements as well as early warning signs of disease or illness.

A practice nurse’s role in providing health assessments often includes but is not limited to:
• documenting current health issues
• updating medical history
• taking measurements
• collating information regarding referrals and the involvement of other health professionals in providing care to the individual
• checking medication use and compliance
• conducting assessments using current and evidence based assessment tools (eg. nutrition, falls risk, mobility and brief mental health assessments)
• providing health education.

Available assessments include:

  • Health assessment for people aged 75 years and older
  • Health assessment for people aged 45-49 years who are at risk of developing chronic disease
  • Type 2 diabetes risk evaluation for people aged 40-49 years
